Review your results/plan in the patient portal or at follow-up.
Start any recommended lifestyle or medication changes as discussed.
Schedule referrals (eye, dental, colon, etc.) if needed.
Reach out with new or worsening symptoms anytime.

Do I need labs? Based on your age, risks, and prior results.
Vaccines? We’ll review and update per guidelines.
How often should I come? Typically yearly, sooner if managing conditions.
Can we discuss multiple issues? Yes—tell us priorities so we allocate time well.
